Update Your Health History
If your medications, health conditions, or allergies have changed since your last visit, please update your dental and medical history before your next appointment. Keeping this information current helps our team provide safe, appropriate care.
Complete the form below, or let our team know at the start of your next visit and we can update your records in the office.
Staying on track
Our recall system
We contact patients when they're due for a recall appointment, typically every six months, though your dentist may recommend a different interval based on your individual oral health needs.
If you haven't heard from us, if you'd like to move up your appointment, or if you need to reschedule, please call us directly at 306.374.7272. We're always happy to help find a time that works.
Keeping up with regular checkups and cleanings is one of the most effective ways to catch small issues before they become larger ones, and to maintain the oral health you've worked to build.
Recall reminders
We send reminders by text or phone when you're approaching your next due date. If your contact information has changed, let us know so we can keep your records current.
Reschedule or cancel
If you need to change an existing appointment, please call us as soon as possible so we can offer the time to another patient and find a new slot for you.
Dental concerns between visits
If you develop a toothache, notice swelling, break a tooth, or have any other concern between regular appointments, call us. We'll do our best to see you promptly.
Post-visit instructions
Not sure what to expect after a filling, extraction, crown, or cleaning? Our post-visit instructions page covers what's normal and when to call us.
