Collecting complete and accurate health information allows us to provide safe, informed dental care from your very first appointment.
Why we ask about your health
Dental care does not happen in isolation from the rest of your health. Your current medications, known allergies, systemic health conditions, and other medical information all have a direct bearing on how we assess, plan, and deliver your dental care.
For example, certain medications can affect bleeding, gum health, or how the body responds to dental materials. Some systemic conditions, such as heart conditions, diabetes, or immune-affecting medications, may require modifications to standard dental procedures. Knowing your health history helps us provide care that is safe and appropriate for you specifically.
This is why we collect a health history from every new patient, and why we ask all patients to let us know of any changes at each subsequent visit.
New patient health history form
As a new patient, you'll be asked to complete a health history form before your first appointment. This form asks about your medical history, current medications, allergies, and any dental concerns or previous dental experiences that may be relevant to your care.
Complete the form below before your first visit, or arrive 10 minutes early and complete it at the clinic.
Existing patients
If you're an existing patient, we'll ask you at each visit whether anything has changed since your last appointment, including new medications, diagnoses, allergies, or other health updates. Keeping your health information current allows us to continue providing appropriate care.
From time to time, we may also ask you to update a full health history form, particularly if there have been significant changes or if it has been some time since your records were last reviewed. You can find the Medical History Update form on the Existing Patients page.
Privacy & confidentiality
All patient information collected at Wildwood Dental Clinic is handled in accordance with Saskatchewan's privacy legislation and the federal Personal Information Protection and Electronic Documents Act (PIPEDA). Your patient records are confidential, stored securely, and accessed only by members of your care team. We do not share your personal health information without your consent, except as required by law.
Forms for specific procedures
For certain procedures, such as sedation, surgical extractions, or other treatments requiring informed consent, additional forms may be used. Your dentist or clinical team will walk you through any procedure-specific documentation before treatment begins and will take the time to answer your questions.
