Space Maintainers

When a primary tooth is lost earlier than expected, a space maintainer can hold the gap open so the permanent tooth has room to erupt in the correct position.

Why space matters when a primary tooth is lost early

Primary (baby) teeth do more than allow children to chew, they act as placeholders that guide the developing permanent teeth into the correct position as they erupt. When a primary tooth is lost before its time, whether due to decay, injury, or extraction, the teeth on either side may gradually drift into the empty space.

If this happens, the permanent tooth that was meant to occupy that position may not have enough room to erupt normally. The result can be crowding, misalignment, or impaction of the incoming permanent tooth, issues that may require more involved treatment later.

What a space maintainer does

A space maintainer is a passive appliance, it does not move teeth, but simply holds the gap open so that adjacent teeth cannot drift. It is custom-made to fit the child's mouth and is removed once the permanent tooth is ready to erupt into that space.

Types of space maintainers

Band and loop (fixed)

The most common type for children. A metal band is cemented around an adjacent tooth, and a wire loop holds the space open. Fixed maintainers stay in place without the child needing to manage them.

Removable

Some older children may be candidates for a removable appliance, similar in concept to a retainer. These require the child to be cooperative about wearing and caring for it consistently.

Does every early tooth loss require a space maintainer?

No. Whether a space maintainer is needed depends on several factors: which tooth was lost, the child's age, how much of the jaw has already developed, and how close the permanent tooth is to erupting. We assess each situation with x-rays to see where the permanent tooth is and make a recommendation based on what we find.

For example, if a primary tooth is lost close to the natural time when it would have fallen out on its own, and the permanent tooth is close to the surface, a maintainer may not be necessary.

What to expect and how to care for it

Space maintainers are checked at each regular dental appointment to confirm they remain properly positioned. They are removed once the permanent tooth begins to erupt into the space.

Care at home involves regular brushing around the appliance and avoiding sticky or chewy foods, caramel, gummy candies, and similar items can dislodge or distort the appliance. If the maintainer feels loose or uncomfortable, contact us promptly so we can assess it.
